All clinical trials must be conducted under regulations and guidance of the Columbia University and New York-Presbyterian Hospital. All investigations involving humans requires review and approval of the Institutional Review Board (IRB).

All clinical trials require review by the Clinical Trials Office. Clinical trials that receive any extramural support, including drugs or devices, require a contract that is reviewed by Columbia University legal counsel and is signed by authorized representatives of both Columbia University and the New York-Presbyterian Hospital.

The conduct of clinical trials without these reviews and approvals may create ethical and legal liability for the principal investigator.

I, the Principal Investigator, agree that I have examined this submission, accompanying schedules, and statements. To the best of my knowledge and belief, they are true, correct, and complete. In addition, if discrepancies are identified at a later point, I agree to collaborate with all parties involved to the point of resolution.

I understand that this proposal is subject to both Hospital and University policies, which, among other things, prohibit deficit accounts.
